The Connections Program for Young Parents

who we are

The goal of The Connections Program is to link young parents in the community with services that may help.

A Community Health Nurse from The Connections Program visits young parents during pregnancy or in the hospital at delivery. We connect young parents with needed resources. The program includes the agencies listed here:

• Multnomah County Health Department
• Insights Teen Parent Program
• NE YWCA
• North Portland Youth and Family Center

We also link young parents to other programs in the community, including programs for young parents who are non-English speaking.

• home visits by a Community Health Nurse
• parenting classes and support groups
• education/job training
• information about your baby's growth and development
• family planning and birth control
• childcare information
• Women, Infants, and Children (WIC) a food supplement program and nutruition education program
• We can also help you with:
• diapers
• food
• housing resources
• transportation
