who we are

The goal of The Connections Program is to link young parents in the community with services that may help.

A Community Health Nurse from The Connections Program visits young parents during pregnancy or in the hospital at delivery. We connect young parents with needed resources. The program includes the agencies listed here:

We also link young parents to other programs in the community, including programs for young parents who are non-English speaking.
